As a board certified family practice doctor specializing in integrative medicine and weight management, my relationship with Semaglutide, known commercially as Wegovy and Ozempic, is a complex blend of admiration and caution. Semaglutide, part of the GLP-1 receptor agonist class, has been revolutionary in treating obesity and type 2 diabetes. By mimicking the GLP-1 hormone, it enhances insulin secretion, inhibits glucagon, and effectively suppresses appetite. These mechanisms make it a powerful tool in achieving weight loss and improving glycemic control.

However, it is important to acknowledge the potential side effects and concerns associated with Semaglutide. Common side effects include nausea, vomiting, diarrhea, and constipation. More serious concerns involve the risk of thyroid C-cell tumors, including medullary thyroid carcinoma. While these risks were primarily observed in rodent studies, they have raised concerns about long-term safety in humans. Consequently, patients with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 (MEN 2) are generally advised against using this medication.

Recent media reports suggest that patients might need to remain on this medication indefinitely to sustain weight loss. I challenge this narrative, particularly for the majority of non-diabetic patients. The issue lies not in a genetic defect for most, but in long-standing lifestyle and dietary habits. Consider an analogy: if muscle pain due to a body imbalance is only treated with advil without addressing the underlying cause, the pain will resurface once the medication is stopped. Similarly, GLP-1s can assist in weight loss, but without addressing the root causes—such as poor nutrition and sedentary lifestyle—the benefits will likely not last.

To elaborate, GLP-1 receptor agonists like Semaglutide work by mimicking the action of the naturally occurring hormone GLP-1 (glucagon-like peptide-1). This hormone is released in the gut in response to food intake and plays a crucial role in regulating blood sugar levels by enhancing insulin secretion and inhibiting glucagon release. Additionally, it slows gastric emptying and promotes satiety, making individuals feel full longer and reducing overall food intake. These effects collectively contribute to significant weight loss and improved metabolic health.

Despite these benefits, the underlying issue for most individuals is not a genetic anomaly but rather acquired habits and lifestyle choices. Many people fall into a vicious cycle of “metabolic syndrome”, where excess adipose tissue perpetuates hormonal imbalances, further promoting fat storage. Over time, poor dietary choices and lack of physical activity become ingrained habits, leading to weight gain and associated health problems. While GLP-1 agonists can act as a temporary metabolic reset, sustainable change requires a fundamental shift in lifestyle and behavior.
